Requesting Various Documents

Requests cannot be submitted over the phone or by e-mail.
Documents can be requested in the following ways:

  • Use of Automated Certification Machine(only for Registration Certificate, Certificate of Expected Completion,Transcript of Academic Record(Japanese), and Student Discount Certificate)
  • In person at School Affairs Section
  • Regular mail
  • Fax (only in cases where requested documents will be picked up in person at School Affairs Section)

 

Requests Via Regular Mail

 

Send the following to the address listed below.

(1) "Request for Documents" form (Regular Mail version)
(2) Self-addressed stamped envelope
(3)Copy of Student ID Card (For those who have graduated, a copy of government-issued identification will suffice.)

Requests via fax can be received only when documents will be picked up in person at School Affairs Section.

Requests In Person

 

When requesting documents in person at School Affairs Section, look for, complete, and submit the "Request for Documents" form.

"Days" Monday - Friday (excludes Saturdays, Sundays, national holidays, year-end and New Year holidays, and summer holidays)

"Hours" 9 AM - 5 PM

"Location" Office of the Registrar, Graduate School of Frontier Sciences (First Floor, Transdisciplinary Sciences Building)

Items to Bring When Picking Up Documents at School Affairs Section

 

Self---Student ID Card or official identification
Proxy---official identification and proxy letter (no specific format, signature/seal of requestor necessary)

 

Processing Time

 

Japanese---Documents are ready 2 days after requests are received (excluding Saturdays, Sundays, and holidays).
English---Documents are ready 7-10 days after requests are received (excluding Saturdays, Sundays, and holidays). (The above times apply when requests are received in person at School Affairs Section.)

*Requests via regular mail will require additional processing time, so allow plenty of time for processing.
